To register your team please complete and submit the form below then return and pay via the Paypal button or directly into Touch4Christchurch bank Account
Deadline for Team nominations are 5pm 24th April 2011
Cost:
*$350 per Corporate Team – Mixed grades only
*$250 per Premiers, Open A/B and Masters Teams per grade regardless of age (Premiers are equivalent to elite, Opens are social grades, Masters range from 27's womens to 45's mens womens, mixed)
*$150 per Youth Team
*$100 per Junior Team
FREE under 8 teams (mini touch match of 4 x 5 minute quarters)
$77 compulsory payment toward insurance is included in team registrations.
$33 (Junior/Youth Teams) compulsory payment toward insurance is included in team registrations
Individual players not in a team please contact me and you will be contacted for team placement.
We have a number of single players available that are looking for a team to join into for this event.
If your team is experiencing a bit of a shortfall in terms of team numbers, we may be able to help you out with boosting your numbers, please email me at claudiakumeroa@hotmail.com with your grade, how many players you need and the gender of the players you need and we will try to find a player(s)

It is preferred that you deposit entry costs into the T4C bank account due to paypal charging me banking fees, meaning less monies go toward the appeal but if it's your preferred form of payment that's more than fine.
